If you have any reason to suspect a breach has taken place, take the following actions immediately and in this order:

  1. Call the following until you’ve established human contact:  your Manager, re|solution's Privacy and Security Officer, re|solution's IT Manager.
  2. Take all reasonable steps to mitigate the impact of the breach. 
  3. Be prepared to answer the following questions:

-  What is the nature of the information you believe has been compromised?

-  Was it for one or more facility?

-  (if applicable) Roughly how many patients’ data might be involved?

Do NOT contact any potentially impacted facilities.  Any contact authorized above will do so.
